AI Appointment Reminder Calls That Confirm or Reschedule on the Call

Appointment reminder calls for health systems, multi-location practices, and service businesses, and for the contact centers and BPOs that run their outbound lines. An AI voice agent calls before the visit, checks that it reached the person it called, states the date, time, and place, and confirms, reschedules, or cancels on the same call. Unanswered contacts are retried on your schedule and can be followed up by text, and each outcome is logged per contact.
Trusted by over 4,000 businesses powering 100+ million calls per month.
How It Works

Set up AI appointment reminder calls in four stages, from the reminder script to campaigns running on your own numbers.

Write the reminder for the phone
Write what the agent says and which details it may share, keep the appointment time, location, and provider in contact fields the agent reads as variables, and connect the calendar it books into: a native integration such as Cal.com or Calendly, or your EHR, practice management, or field service system through a custom function.
Test on simulated reminder calls
Run simulation tests with a patient who confirms, one who needs a new time, one who cancels, a family member who answers instead, and someone who asks not to be called again, and grade every run before a real contact is dialed.
Launch an outbound call campaign
Upload the coming days' appointments as a CSV or XLSX file, or filter contacts synced from your CRM, pick the compliance checks, set attempts, delays, and a calling window in each contact's local time, and choose caller IDs.
Track outcomes per contact
Each campaign counts contacts dialed, connected, and converted, charts outcomes such as confirmed, rescheduled, cancelled, and do not call, and opens each conversation's transcript and recording.
Use Cases

Reminder calls that end with a confirmed, moved, or cancelled visit

Confirm, reschedule, or cancel on the same call

Need every reminder to end with an answer, not a voicemail nobody returns. The agent checks it reached the person it called, states the date, time, and location, and asks if the visit still works. A yes is logged as confirmed. A caller who needs a new time hears open slots and is rebooked on the call: Cal.com has a native reschedule tool; with Calendly, the agent cancels the booking and books the new time; and EHR or practice management schedules connect through a custom function that calls your API. Start from the reminder and no-show reducer template.

Reminders timed to the visit and the contact's clock

Need reminders that land when they help. A common pattern is a list sent a day or two ahead: upload the coming appointments, or filter contacts synced from your CRM, and launch a campaign. Attempts run on delays you set in minutes, hours, or days, inside the calling window you set, by default in each contact's local time, skipping the holidays you select. With local presence dialing, each contact sees a number from its own area when you have one there, and the same number on every call and text. For a reminder tied to an exact time, your scheduling system can place the call through the API instead. Pine Park Health's reminder agent calls patients or their responsible parties two days before each visit.

Diagram of one reminder contact in an outbound call campaign. If nobody answers: attempt 1 calls now with one immediate redial and hangs up on voicemail, attempt 2 runs 4 hours later, attempt 3 the next day leaves a voicemail, then a text follow-up handles the reply. If the contact answers: confirmed ends the sequence as a success, needs a new time rebooks through a calendar tool, cancels ends the sequence, and asks not to be called marks the contact do-not-call.
No answer: retries, voicemail, then a text

Need unanswered reminders to keep working without a person redialing. With double dial on, an unanswered call gets one immediate second dial, and the next attempt follows after the delay you set. A voicemail counts as unanswered even when the agent leaves one, so the docs recommend hanging up on early voicemails and leaving a message only on the last attempt or two. When the last attempt goes unanswered, the contact can move to an SMS step, where a chat agent texts the reminder and handles the reply, from numbers registered for texting. For what a healthcare voicemail should leave out, see our guide to HIPAA appointment reminders.

No-show recovery while it's fresh

Need missed visits rebooked while the patient still remembers why they booked. When your system marks an appointment as missed and tags the contact, an Auto-enroll campaign enrolls the contact, its compliance checks usually finish within about five minutes, and the agent calls in the next calling window to offer a new time. A rebooked visit can write a value such as rebooked to a contact field, which syncs to your CRM when that field is mapped. A contact enrolls in a given campaign only once, so a standing no-show program runs as a new campaign per period, duplicated from the last.

Recall and care-gap calls with a live handoff to the office

Need overdue patients booked, not just reminded. For recall and preventive-care lists, the agent confirms identity, says in general terms what the patient is due for, and collects a preferred day and time. Where the booking lives with a provider's office rather than a calendar the agent can reach, an agentic warm transfer has a second agent call the office while the patient holds, talk to whoever answers, and, if the office is ready, connect the patient so staff can book the visit on the same call. Visit prep: who should be there and what to bring

Need the visit to go ahead once the customer shows up. For in-home service visits, the agent confirms the arrival window, that the people who make the decision will be home, how long the visit takes, and what it covers. For appointments that need paperwork, it reads back what is still missing and when it is due, and can text the list from your number once it is registered for SMS. Trade and dealership teams can pair this with inbound booking, such as HVAC service booking and dealership service scheduling.

Built for Contact Centers, Not Demos

Retell is the AI Voice Platform Enterprise Contact Centers Run On

Retell lets you build, test and deploy AI phone agents that handle real call volume, so your team spends its time on the conversations that need a person.
SOC 2 Type II and HIPAA compliant. Trusted by teams running 1.000+ calls a day.
Compliance

Reminder calls carry names, dates of birth, and appointment details, and outbound calling has its own rules.

HIPAA

Retell is HIPAA compliant, with a Business Associate Agreement you can self-sign at no extra fee. Sign it before any protected health information passes through Retell.

Do-Not-Call Screening

Campaigns can screen each contact against your internal do-not-call list, federal and state registries, known TCPA litigators, reassigned numbers, and line types before the first dial. Screening skips numbers you shouldn't call. It doesn't confirm consent.

PII Redaction

Pick the categories to scrub per agent, such as names, dates of birth, medical IDs, and addresses. Scrubbing covers transcripts, recordings, and tool call data, and keypad digits are masked too.

Data Retention

Set per agent how long transcripts, recordings, and logs are kept, from one day to two years, after which they are deleted automatically.

FAQ

Frequently asked questions.

What are AI appointment reminder calls?

They are outbound calls in which an AI voice agent reminds a patient or customer of an upcoming appointment and handles the answer on the same call: it confirms the visit, offers new times and rebooks, or cancels. It is not a parallel dialer, which rings several lines for one human rep: each reminder is a full agent conversation. On Retell AI, reminder lists run as outbound call campaigns with retries, calling windows, and do-not-call checks, and each outcome is logged per contact.

What should an appointment reminder call script say?

Keep it short: who is calling and from where, a check that the intended person answered, the date, time, and location, and one question, whether they can still make it. Then branch: confirm, offer two or three new times, or cancel after checking once more, and close with a callback number. In healthcare, share only what the reminder needs, as our guide to HIPAA appointment reminders explains. The reminder and no-show reducer template has a working version of this flow.

How often do patients miss appointments?

In a study of 10 clinics at a large Veterans Affairs medical center over 12 years, the mean no-show rate was 18.8%, and the average cost of a no-show was $196 in 2008. In a year-over-year test, a reminder-letter system made no significant difference to the no-show rate, and the authors concluded that reminders only modestly reduced no-shows, which imposed a major burden on the health system. Are AI appointment reminder calls allowed under the TCPA?

In February 2024 the FCC confirmed that AI-generated voices count as an artificial or prerecorded voice under the TCPA, so these calls need the called party's prior express consent unless an emergency purpose or exemption applies, and prior express written consent when the call is telemarketing. Getting and recording consent is the caller's responsibility. Retell campaigns screen contacts before dialing and ask you to confirm consent at launch, but they don't verify it, and they don't enforce legal calling hours. This is not legal advice. See our TCPA guide for outbound AI voice calls and check your program with counsel.

What happens when the patient doesn't answer?

The call is redialed once right away if double dial is on, then the next attempt runs after the delay you set, inside the calling window. Voicemail counts as unanswered even when the agent leaves a message, and each attempt has its own voicemail setting. After the last attempt, the contact can move to a text step or exit. A contact who calls the campaign number back within 30 days, while still waiting for an attempt, reaches the reminder agent. The sequence docs cover the details.

Which calendars and systems do reminder calls connect to?

Native calendar integrations include Cal.com and Calendly, for checking availability, booking, and cancelling; Cal.com can also reschedule in place. HubSpot, Microsoft Dynamics 365, Zoho CRM, and the other native CRM integrations sync contacts into Retell, and outcomes written to mapped fields sync back. Systems without a native integration, such as most EHR, practice management, and field service systems, connect through custom functions that call your API during the call. Lists can also come from a CSV or XLSX upload of up to 50,000 rows per file.

Can a contact center or BPO run reminder calls for several clients?

Yes. Give each client its own agent, script, caller IDs, and campaigns. Contacts are keyed by phone number in a workspace, a contact that matches two campaigns is called by both, and a do-not-call mark is workspace-wide, honored by every campaign with internal do-not-call screening on, so keep each client's campaigns on its own tag, or run each client in its own workspace to keep contact data and do-not-call marks apart. Campaign calls share your workspace concurrency with other outbound calls, and an inbound reserve keeps room for inbound lines. See AI customer support for BPOs.
